Abstract

Introduction: It is very important to use unified rehabilitation forms and documents in multidisciplinary and long-term process of rehabilitation of children with cerebral palsy (CP).

Aim of the paper: The paper presents a scheme of documentation for children with CP which is necessary for members of a rehabilitation team. This document consists of two parts. The first part is completed by a physiatrist - conjoint rehabilitation form consisting of information necessary for all specialists working in a team. The second part contains several documents and is filled by every member of the rehabilitation team. The authors propose a structured way of documenting the patient’s condition and his/her needs for rehabilitation according to ICF.

Conclusion: According to current standards in dealing with CP children, members of the therapeutic team should employ consistent documentation in all rehabilitation centers while planning multidisciplinary rehabilitation.
