Tethered cord syndrome – variety and characteristic pictures in the MR examinations of the thoraco-lumbo-sacral spinal column

The autors present the analysis of 36 cases tethered cord syndrom examined within the years 1999-2003 in MR Department of Provincial Hospital No.2 in Rzeszów , with applying MR 0.5 T Elscint Privilage scaner. The results of other examinations like KT, RTG ,USG in these patients were also presented. The characteristic symptoms of tethered cord syndrom in MR imaging and heterogenity of coexisting with tethered cord other malformation of nervous, skeletal et genitourinary systems were defined. It was also demonstrated, that among 35 patients with diagnosed tethered cord syndrom 2/3 of examined demonstrated also developmental abnormalities of spinal cord and meninges in form of : hernias (14) and lipomas (10). Coexistance of other malformations in spinal cord and dural sac in 12 patients (in 4 patients double pathology, in 1 patient triple pathology) such as hydromyelia, diastomatomyelia, diplomyelia, syringomyelia and arachnoid cyst and also brain malformation (hydrocephalus in 6 patients and Chiarie malformation in 2 patients) were also defined. Additional examinations demonstrated coexistance of malformation in urinary system in 13 patients : neurogenic bladder (11), pseudodiverticula of urinary bladder (6), reflux to ureters (4), hydronephrosis (2) and ren duplex (2). It was also demonstrated ,that tethred cord syndrom is rarely occured pathology of nervous system, mainly concern children and female. The course of it can be present without clinical symptoms and as isolated disease. Mostly concerns people with congenital anomalies of lumbosacral vertebral column and final segment of spinal cord. It is often associated with other spinal cord and brain malformations and urinary system anomalies. First choice examination in imaging of tethered cord syndrom is magnetic resonance, and computed tomography as complementary method, which present better the nature of skeletal system malformations.
