The study describes the indications, advantages and complications of the buccal fat pad flap and reports our clinical experience with the flap for intraoral reconstruction after the prior failures. Methods. From 2001` to 2006`a pedicled buccal fat pad flap was used to reconstruct oral defects in 16 patients, prospectively analyzed. The buccal fat pad was frequently used for the closure of oro-antral fistulas. Results. Adequate closure of the defect was achieved in all cases. The buccal fat pad healed without complications in 11 patients, whereas in 5 patients there was a partial loss of graft or some retraction of the buccal fat pad. Conclusions. It is an acceptable type of reconstruction. This procedure of closure oro-antral fistulas has wide application after primary failures of reconstructions and a high degree of success.
